New Zealand Medical Employment Documentation Hi there. Below you will find the forms we require for employment placements in New Zealand, e.g. speech and language therapists, physiotherapists and occupational therapists.

Please take a minute to read this - yes, we know it's a pain but it will save us all some time in the long run.

We had to make a few choices to keep the site functional (visually distracting, slow to load - that's the sort of stuff we wanted to avoid) so we had a few tough calls to make. As a result we have:

Provided both PDF and Word 2000 files
Formatted the paper for A4 size

Now to the action part:

Choose the format that works for you - either PDF or Word.
You will need to download the MSI Application Form. The information in this will provide us with a good baseline for our working relationship.
There is also a CV template in MS Word format that you might find useful.
