Hi there. Below you will find the forms we require for employment placements in New Zealand, e.g. speech and language therapists, physiotherapists and occupational therapists.

Please take a minute to read this - yes, we know it's a pain but it will save us all some time in the long run.

We had to make a few choices to keep the site functional (visually distracting, slow to load - that's the sort of stuff we wanted to avoid) so we had a few tough calls to make. As a result we have:

Provided both PDF and Word 2000 files
Formatted the paper for A4 size

Now to the action part:

Choose the format that works for you - either PDF or Word.
You will need to download the MSI Application Form. The information in this will provide us with a good baseline for our working relationship.
There is also a CV template in MS Word format that you might find useful.
